Referral Process
Our staff are available to take referrals for outpatient therapy or community-based services Monday-Thursday 8:30am - 6:00pm and Friday 8:30am-5:00pm. If you would like to make a referral or simply have questions or concerns regarding mental health services, please call (804) 447-6382.
Outpatient Therapy:
Submit your information through our secure portal by clicking on the link below- “request an appointment for outpatient therapy”. Someone from our team will contact you within 48 hours. If you have not heard from anyone within 48 hours, please do not hesitate to contact the office manager at (804) 447-6382 ext. 401. If you are a provider referring someone, we will notify you that we have received the referral and any relevant information.
Community-Based Services:
Access the referral form below by clicking on the link “referral for community-based services”. Once you complete the referral form, please email it to [email protected] or fax it to (804) 447-6383.
